On Tuesday, Nov. 22, 2022, Captain Basil Pizzuto was conducting Self Contained Breathing Apparatus (SCBA) refresher training to fellow members of the department. The following morning, Wednesday, Nov. 23, 2022, while at his full-time job, he suffered a heart attack and coded. EMS personnel arrived at the scene and were able to revive him.

Penn Hills EMS and White Oak EMS identified the EMS provider who died after a heart attack and ambulance crash as Paramedic Nick Theofilis. He worked full-time for Penn Hills EMS and part-time for White Oak EMS, where he previously worked as an EMT.

*The $5,000 survivor benefit is paid when a line of duty death occurs. Line of duty is defined as AFBA members who are Emergency Service Providers to include law enforcement, firefighters, and emergency medical services providers who are killed while involved in operations at or responding to/from the scene of an incident. If death occurs otherwise, outside of a suicide, the beneficiary will receive a $2000 benefit.
